Holly Parkey, RN provides basic School Nurse Services and Special Education updates for all students in USD 353, Wellington. Ms. Karolyn Lacey, Health Aide works with the School Nurse. E-mail the Nurse directly if you have questions about your students health needs at school, immunizations, or screenings provided at school.
Medication Administration at School
Any Prescription or Over the Counter medication that is to be given to or taken by your student while at school or during a school sponsored event/activity is required to have an Annual Medication Order & Parent/Guardian release/request on file, prior to the medication being taken. The medication order form is available on this webpage. (below) It is not required that this form be used. Elements required are - Dr./Dentist order for the medication, Name of Medication, Dosage, Frequency, Reason for Medication, Signature of Dr./Dentist, and Parent/Guardian signature/request to administer medication.
Full explanation is printed in the handbook for each grade level & copies of the Medication order sheet are available at all school buildings.
Free Dental Screenings will be provided during the school year (usually during the fall semester). Permission slips will be included in enrollment packets. Parent(s)/Guardian(s) for each student will have the opportunity to have their student participate in this free screening which is provided by local area dentists during the school day. Students will not be screened unless a permit is signed.
Students will be given a short report form to bring home following the Dental Screening.
Student's Immunization records or updates to their records are due in the Health Office of the school they attend at enrollment or when boosters are received/required. Records are reviewed for KG, New Students & those who are due for required boosters annually. Exclusion dates will be set for students who do not provide proof of immunizations received & parents/guardians will be notified of needed Immunizations & exclusion dates
